Job Summary:
Primary functions are to administer skilled nursing care for patients/clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patients/clients and their families, and a referring agency. Assumes the responsibility for and participates in the coordination of care.
Home Health Care experience is preferred but we are willing to train if you do not have any Home Health experience.
As a Registered Nurse, you will:
- Work one-on-one with patients in a home environment
- Computerized documentation using Axxess Technology Solutions. (We will train you in using the software if you have no experience.)
- Provide skilled care to the patient as per physicians’ orders and by instruction to LVN’s, Home Health Aides, and other qualified staff.
- Perform Responsibility for home health Patient Admission, Plan of Care, Re certification, Resumption and Discharge Oasis.
- Exercise strong decision making and judgment skills
- Registered Nurses work with an interdisciplinary team in weekly case conferences
Qualifications:
- Registered Nurses must be licensed in the State of Texas.
- Diploma, Associate, or Baccalaureate degree in nursing
- Minimum of one year Home Healthcare experience preferred.
- Must be CPR certified
- Must work well independently, enjoy problem solving and understand the importance of accurate documentation.
- Must have critical thinking skills.
- Must have exceptional interpersonal skills.
- Valid Driver’s license and proof of automobile insurance.
