We are seeking a dedicated Behavioral Health Professional to join our team. The ideal candidate will provide mental health counseling and support to individuals in need.
The ideal candidate will have experience in providing counseling and therapy services to individuals, families, and groups dealing with various mental health issues
As a Behavioral Health Provider, you will be responsible for conducting assessments, developing treatment plans, and providing therapy sessions to clients. You will work closely with other healthcare professionals to ensure comprehensive care for clients.
Responsibilities:
· Effectively utilize a brief consultation model to communicate with the primary care provider. Use a team-based care approach that supports patients and their families in self-management, self–efficacy and behavior change.
· Provide consultation and training to other members of the primary care team in mental health, chemical dependency, and behavioral medicine. Enhance understanding of the patient, provide decision support for treatment planning, and assist in the implementation and monitoring of biopsychosocial treatment plans.
· Consult with primary care providers/team to manage psychosocial aspects of chronic and acute diseases and manage patients with chronic mental medical problems efficiently and effectively. Working as a primary care team member to develop specific clinical pathways or best practice programs for targeted patient groups. Identify referral resources and facilitate referrals for further assessment and additional services as appropriate.
· Provide behavioral health services and substance abuse screening to children, adolescents, adults, couples, and families to identify maladaptive coping strategies, and develop harm reduction strategies.
· Perform comprehensive bio-psychosocial assessment for mental health, substance abuse, co-occurring disorders, domestic violence, and medical needs. Conduct follow-up as appropriate based on results of evaluation, medication adherence counseling and disease self-management counseling.
· Utilize electronic medical records to document all patient/ caregiver interactions, as well as other electronic tracking tools and measures.
Qualifications:
· Counseling, Psychology, Social Work, or related field.
· Valid license, Independent to practice as a Behavioral Health Provider (LPCC, LISW).
· Three or more years of related experience with population-based healthcare principles and excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
· Previous experience working within ethnically and culturally diverse families.
· Strong communication and interpersonal skills.
· Ability to work effectively both independently and as part of a team.
· Compassionate and non-judgmental approach to working with clients.
