We are looking to hire a Bilingual Spanish Speaking Case Planner to work in the Bronx Prevention Program. Must be fluent in Spanish (reading & writing).
Major Duties:
- Conduct a face to face contact with newly admitted youth and their treatment parents, within 72 hours and weekly mandated home visits thereafter to provide comprehensive case planning services.
- Conduct mandated face to face home visits/contacts with birth parents and/or discharge resources for the provision of permanency, case planning and discharge planning.
- Complete required paperwork, including but not limited to FASPS, Plan Amendments, Permanency Hearing Reports, life skills assessments and progress notes.
- Keep case records up-to-date with the status of case activity, including progress notes on all contacts.
- Make all court appearances related to cases, representing the agency as to the permanency, safety and well being of participants served by the program.
- Set-up, monitor and document all family and/or sibling visits.
- Provide direct crisis intervention as needed.
- Collaborate with Treatment Parents, Youth, and Socio-therapists around development and implementation of behavior plans and goals.
- Attend and participate in all applicable ACS meetings, transfer conferences, Treatment Team Meetings, Monthly Shared Quarterly Meetings and monthly Staff Meetings
- Coordinate and participate in all applicable Family Team Conferences.
- Attend Preparing Youth for Adulthood Workshops with youth on a rotating basis.
- Gather social history, to assess family strengths and weaknesses, and to assess existing problems to develop an individualized and culturally appropriate service and behavioral plan.
- Work with family and youth to develop the most appropriate permanency plan with emphasis on lasting family connections, which can be, improved family relationships, adoption, mentor , etc
- Work with the Adolescent Services team for the comprehensive service planning for each youth, including education, employment and other PYA skills.
- Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care, when applicable, in the department/program to create a non-violent and trauma-sensitive culture.
- Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture.
- Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment
- Locate resources and make referrals as needed. These include consistent outreach and referrals in the community. These will include but not be limited to medical and mental health, clinical support services, MH crises intervention services, supportive skills, etc.
- Monitor foster parents in terms of their ability to implement behavior plan and meet the needs of the child in their home, and make recommendations regarding future use of the home, in conjunction with the Homefinding Department.
- Support and counsel foster parents in their efforts to care for the child, and include them in the development of the behavioral and service goals and tasks.
- Implement and integrate the principles and procedures of Solution Based Casework: assessment, family and individual level outcomes, action plans and documentation and celebration.
- Actively participate in case consultation, continued SBC training and the SBC certification process.
- Respond to 24 hour on call service when needed.
- Perform other duties as assigned.
Qualifications:
- Bachelor’s degree in related field required.
- Must have experience working with youth and families, prior foster care experience preferred.
- Must be able to engage youth and families and establish working relationships with them to ensure their full participation in their plan.
- Good written and verbal skills necessary.
- Must enjoy working with adolescents.
- We require three supervisory references. Must be able to pass fingerprint clearances.
