JOB SUMMARY
Supervises daily operations of one of the Community Support teams which provides assessment, advocacy, service planning, care coordination and referrals for children and their families who have been referred from DCF for the Differential Response System of Care.
KEY RESPONSIBILITIES
- Provides primary supervision of community support workers and parent navigators assigned to the DCF Region 4 catchment area.
- Provides triage and assignments for all new referrals.
- Supports the orientation of newly hired staff.
- Provides some direct services to children and their families which include:
- Assessment and engagement.
- Formulation and contracting for services or treatment plan.
- Implementation of service plan, modifying it as appropriate during the course of treatment and community support.
- Transition planning, which may include plans for follow-up treatment and/or linkages to community support services.
- Case management.
- Participates in peer consultation and training with other staff regarding case assessment and service planning.
- Works as a team member with other clinical and administrative staff to respond to client and programmatic needs.
- Participates in implementation and integration of Results Based Accountability in day-to-day operations of program.
- Teams with other program and agency staff to ensure integration of services and development of internal system of care to address client’s unmet needs or service gaps.
- Partners with Director to identify areas and develop plans for professional growth.
- Contributes to agency life by participating in agency wide task forces, committees, or decision-making teams as appropriate.
- Contributes to agency visibility and development of community linkages by representing The Village in community groups or committees as requested.
EDUCATION
- Master’s in social work, psychology, marriage and family therapy, child and family development or other related field.
EXPERIENCE
- Must have significant experience with populations served by The Village and “hands-on” experience working with the specific communities related to this program.
- Supervisory, management, and administrative experience, skilled at using validated measures, observation and interpretation of behavior.
- Ability to be a good team player and leader, to demonstrate flexibility in approach to job and others, to be insightful, self-reflecting, and aware of impact on others. Must be patient, empathic, and show good judgment in pressured situations.
- Familiarity, sensitivity, and/or experience working with diverse populations; Bilingual/bicultural skills preferred.
