Founded in 1967, Casa is one of the only treatment centers in Southern California designed to specifically meet the needs of women in recovery. Casa Treatment Center is a residential, outpatient, and sober living substance abuse facility headquartered in Pasadena. Casa is licensed by the State of California Department of Health Care Services and accredited as a behavioral health care program by the Joint Commission.
Job Summary: The Case Manager position reports directly to the Executive Director and provides direct client care and case management services. The Case Manager position is a full-time position Monday to Saturdays, and/or evenings.
Essential Duties
- Case Management: Perform biopsychosocial evaluation, present findings in Case Management meetings and incorporate Medical Director, Clinical Director and other leaders input into treatment plan.
- Client Education: Inform clients about the various tools and skills that will assist them with treatment.
- Client Counseling: Assist client in motivation and self-discovery utilizing clinical interventions. Knowledge of Motivational Interviewing techniques are vital.
- Treatment Planning: Work closely with client to develop individualized treatment plan and SMART goals integrating the client’s strengths, needs, abilities and preferences.
- Care Coordination: Coordinate client care with outside service providers as indicated by client treatment plan.
- Group Facilitation: Design, facilitate and document groups as assigned by program leadership.
- Electronic Health Records (SAGE): Complete and document client progress in the SAGE system as defined by agency Policies and Procedures.
- Crisis Management: The counselor is responsible for assisting clients in distress, guiding toward emotional regulation and stabilization.
- Utilization Reviews: Submits authorizations on a timely manner
- Billing: Have an understanding of Medi-cal and Insurances billing
Qualifications
This position is best filled by individuals who are open-minded, non-judgmental, thoughtful, patient, detail oriented, organized, able to adapt to changing priorities. Experience with SAGE and the Drug Medi-Cal model is required.
Minimum Requirements
The Case Manager must meet one (1) of the following minimum requirements:
- A Master’s Degree in a social service-related field and registered with an AOD certifying organization recognized by DHCS; or
- A Bachelor’s Degree in a social service-related field, AOD certified, and one (1) years of experience providing SUDT services as a certified AOD Counselor; or
- An Associate’s Degree in social service-related field, AOD certified, and three (3) years of experience providing SUDT services as a certified AOD Counselor; or
- AOD certified and more than five (3) years of experience providing SUD Treatment Services as a certified AOD Counselor
- Experience working with DHCS, Drug MediCal and County of Los Angeles Department of Public Health Substance Abuse and Control (SAPC) contracts is preferred.
- Willingness to work a flexible schedule that may include evenings and/or weekends.
- Experience with Electronic Medical Records (EMR) and Customer Management Relationship (CMR) software such as SAPC SAGE Electronic Health, Welligent, AVATAR, AURA
- Ability to maintain ethical and professional boundaries with clients and staff members.
- Ability to work in and adapt to the culture of a multi-cultural, community-based organization that serves a culturally diverse population reflective of Southern CA.
- Proficiency in Microsoft Office Applications including Excel, PowerPoint, and Word.
- Knowledge of HIPAA regulations and guidelines.
- Training in suicide prevention.
- Excellent verbal and written communication skills.
- Strong skills in crisis management and intervention.
- Must be able to work in a team environment.
- Ability to participate and pass Drug/Alcohol testing on a regular basis.
- COVID-19 protocols in place; we always require temperature checks and mask use. Face shields available.
- CPR/First Aid Certification or attain within 30 days.
- Valid CA Driver’s License.
- This position requires that the employee be able to walk up and down stairs and be able to lift 20 lbs.
