Clinical Development Training Specialist is a key position in the organization with a combination of clinical and educational development, implementation, and monitoring responsibilities. The clinical will develop, implement, and manage Medicare compliant training programs and procedures and maintain up to a half-time caseload of patients depending on extensiveness of educational duties. The successful individual will have sufficient experience to ensure client expectations are met, and assure that clinical, quality and satisfaction outcomes are delivered. Clinical Role
The company is seeking an experienced RN who brings clinical expertise in chronic illness, behavioral health, end-of-life care, preventive services for a unique telemedicine model of care. The ideal candidate will have experience with Chronic Care Management (CCM), Principal Care Management (PCM), Remote Patient Monitoring (RPM), Remote Therapeutic Monitoring (RTM), Annual Wellness Visits (AWV), and/or Behavioral Health Integration (BHI). The successful candidate is a team player and compassionate communicator. The candidate should also have experience with Medicare’s defined chronic care management programs and associated CPT billing for the services, as well as experience building disease-specific care plans. You will review daily medication adherence trends across client groups, monitor data against clinical protocols, respond to patient questions on your assigned patient panel, and conduct routine phone and video visits/calls for your assigned panel of patients.
Educator Role
The Clinical Development Training Specialist is an experienced nurse educator responsible for developing, implementing, and monitoring the training materials that reflect the company policies for a remote therapeutic, physiologic, and chronic condition Care Model that includes chronic, behavioral, and transitional care management. You will be responsible for engaging, educating, and tracking clinical team performance of a remote workforce that includes RNs, MAs, and NPs. You will assure the clinical team has the necessary knowledge to successfully execute their clinical duties while practicing empathetic patient engagement and exemplifying the Dayamed values, and document that with competencies that you develop. You will report to the VP of Clinical Operations.
Responsibilities:
- Develop and support educational and training materials for medication adherence remote therapeutic monitoring (RTM), remote physiologic monitoring (RPM) and chronic care management (CCM, TCM, BHI) programs as defined and regulated by Medicare:
- Assure compliance and appropriate clinical documentation with Medicare and Medicaid rules and regulations;
- Provide robust training to the clinical team including: recruitment and onboarding of new staff; use of the application; become a superuser in the EMR system.
- Develop and implement clinical competencies for onboarding new staff and for annual required education.
- Train clinicians in workflows, patient identification, eligibility criteria and discharge criteria; practice HIPAA compliant virtual visits, voice calls and text messaging.
- Analyze and evaluate clinical monitoring and the day-to-day workflow of the clinical staff and broader clinical team.
- Record staffs’ performance based on outcomes and feedback; document and discuss staff performance against company values and behaviors annually;
- Assure that patient care is provided in accordance with clinical guidelines and professional standards; i.e., educate and evaluate the clinical team on individual ability to:
- Assess patients’ physical and mental wellness needs, preferences and abilities and develop plans to improve;
- Work with patients, caregivers and health care professionals to put care plans in place;
- Actively listen to patients’ concerns and provide counseling or intervention as required;
- Record patients’ progress, charting referrals and scheduling referrals back to primary care provider as needed;
- Evaluate patients’ progress and periodically make adjustments as needed.
- Seek treatments or referrals that balance clinical and financial concerns with the family’s needs and the patient’s quality of life.
- Demonstrate capability in resolving escalated issues arising from operations and requiring coordination with other professionals.
- Contribute to the development and improvement of clinical care protocols, policies and procedures.
- Provide input to strategic decisions that affect the functional area of responsibility.
- Foster an environment of continuous iteration and support by:
- Sharing weekly patient stories and improvement areas
- Ensure that the program has continuous improvement and momentum.
Requirements:
- Active Clinical Nurse Educator (CNE) certification
- Bachelor's Degree in Nursing (BSN) from an accredited university.
- Active, unrestricted multistate compact RN license
- Minimum of three to five years full time equivalent experience in direct clinical care
- Minimum of two (2) years full time equivalent experience in role as a clinical nurse education of clinical staff
- Working knowledge of Medicare, Medicaid and commercial carrier reimbursement policies
- Working knowledge of electronic health record systems
- Ability to analyze and interpret clinical data
- Working knowledge of URAC and case management strategies
- Excellent empathetic, active listening and collaboration skills.
- Excellent organizational skills
Preferred:
- Master’s Degree in Nursing MSN Nurse Educator from an accredited university
- Demonstrated leadership experience
- Experience with CCM, PCM, RTM, RPM, BHI, and AWV
- Experience leading clinical operations in care management
