The Case Manager plays a vital role in providing comprehensive support and assistance to individuals and families in need. This position involves assessing women and children needs, developing individualized service plans, and coordinating access to various resources and programs. The Case Manager serves as an advocate and liaison, working collaboratively with women and children and community partners to facilitate positive outcomes and promote self-sufficiency.
QUALIFICATIONS:
- Bachelor’s degree in social work, psychology, counseling, human services, or a related field. At least 2 years of experience in social work, counseling, human services, or a related field.
- Previous experience in case management, social services, or related field preferred. Knowledge of social services systems, community resources, and programs.
- Proficiency in computer applications and case management software.
OTHER QUALIFICATIONS:
- Must be willing and able to work extended hours and weekends as needed.
- Must be able to maintain client confidentiality.
