POSITION: Behavior Health Registered Nurse
SUPERVISOR: Clinical Nurse Manager
SUMMARY: Plays a crucial role in training and mentoring healthcare staff regarding substance use disorders and the use of MOUD as a treatment mode. They are actively involved in patient care assessment, planning, implementation, and evaluation with a specific focus on the use of MOUD for substance use disorder-related healthcare. Additionally, they collaborate with community partners for strategizing care plans for patients with substance use disorder needs. They actively participate in discharge planning by addressing health requirements within other facilities and upon discharge into the community. Effective collaboration and communication with various disciplines across the community are vital aspects of this role.
QUALIFICATIONS: Registered Nurse with current RN license in State of Kentucky. CPR certification required. Two years nursing experience with demonstrated ability to inspire others to excellence. At least one year of chemical dependency experience is desired. Additional nursing experience in behavioral health, extended healthcare and acute healthcare preferred.
ESSENTIAL FUNCTIONS:
· Treatment Planning: Collaborate with MAT/MOUD providers to develop comprehensive treatment plans for patients in residential treatment. Participate in regular case conferences with stakeholders from within the organization and in the community to coordinate services and develop individualized care plans. Assist with referrals, and on occasion provide transportation, to MAT/MOUD providers, in order to facilitate initiation of MAT/MOUD while the patient is in residential treatment.
· Patient Support: Provide support for patients transitioning from residential to outpatient care, ensuring continuity by sharing medical history and treatment progress.
· Medication Management: Routinely monitor patient responses to medications, address concerns, and ensure compliance.
· Education and Training: Facilitate education to promote best practices and increase understanding of medication-assisted treatment or medication for opioid use disorder, among staff, patients, and community partners.
· Quality Improvement Initiatives: Engage in quality improvement efforts to enhance care delivery and patient outcomes by utilizing data and implementing evidence-based interventions.
· Outreach and Community Engagement: Conduct outreach activities to raise awareness and reduce stigma associated with addiction, participating in community events and collaborating with local organizations.
· Event Management: Manage a forward calendar of scheduled meetings and events with community partners to discuss referral opportunities and strengthen relationships.
· Assists in other Transitions’ programs as needed or assigned.
