Your Role:
Compassionate Admissions: Conduct initial assessments and admissions for new hospice patients on weekends.
Empathetic Support: Provide emotional support and education to patients and families about hospice services, care plans, and end-of-life options.
Collaborative Care: Coordinate with physicians, social workers, and other healthcare professionals to ensure seamless transitions and holistic care.
Clinical Excellence: Administer medications, manage symptoms, and uphold clinical standards with compassion and attention to detail.
Documentation: Maintain accurate and timely documentation of admissions assessments and care plans.
Qualifications:
Nursing Expertise: Current RN license in Virginia, with experience in hospice, palliative care, or a related field preferred.
Compassionate Care: Strong interpersonal skills and a compassionate approach to patient care.
Weekend Availability: Availability to work weekends is essential.
Team Player: Ability to collaborate effectively within a multidisciplinary team.
Detail-Oriented: Strong organizational skills and attention to detail.
Tech Proficiency: Familiarity with electronic health records (EHR) systems is a plus.
