Organizational Summary:
Soldier On is a private non-profit organization providing services for veterans that are homeless or at risk of becoming homeless. Soldier On operates multiple transitional and permanent housing residencies in Massachusetts, providing housing for nearly 400 veterans per night. In addition, Soldier On has been the recipient of three SSVF grants since September 2011. The SSVF program is a grant funded by the VA awarded to private non-profit organizations and consumer cooperatives. These grants provide case management services, outreach services, and temporary financial assistance for veterans that are homeless or at risk of becoming homeless. These grants currently serve 50 counties in Upstate New York, 9 counties in New Jersey, 23 counties in Western Pennsylvania, and 4 counties in Western Massachusetts.
Position Summary:
Position is responsible for the coordination and implementation of services to veteran and/or veteran’s household, with goal of services being long-term independent housing stability. Veteran may be residing in Soldier On managed transitional or permanent housing or may be homeless/at-risk of homelessness within the community. The majority of job activities are performed within shelters, social service agencies, veteran’s apartments, and other locations deemed appropriate. The position reports to the Field Supervisor.
Qualifications:
· Relevant education and/or experience in human services or related field.
· Valid driver’s license required.
· Ability to drive up to 6 hours per day if necessary.
· Ability to be supportive and respectful to all veterans served.
· Ability to work in a variety of settings with culturally diverse families.
· Ability to be culturally sensitive and appropriate.
· Capability to work both independently and as part of team.
· Possess strong written and oral communication skills.
· Strong level of understanding and comfort with various forms of technology.
Responsibilities:
· Determine and assess eligibility requirements.
· Conduct intake assessments.
· Assess functional and social needs of veterans to develop a housing stabilization plan.
· Develop and maintain relationships with housing and community service providers.
· Determine and access appropriate Veterans Administration, community, state, and federal benefits and resources.
· Provide case management and follow up services in response to veterans needs on an ongoing basis.
· Maintain client confidentiality.
· Provide support services in crisis situations appropriately and quickly.
· Assist veterans in obtaining and maintaining permanent housing.
· Document and maintain accurate information in client files, including progress notes on all home visits and contact, on a regular basis.
· Knowledge and understanding of grant guidelines.
· Adhere to agency policy, procedures, and professional code of ethics.

Pay: $52,000.00 per year
