Job Summary:
As a Programs Case Manager, you will play a crucial role in our organization by providing comprehensive support and assistance to individuals participating in our programs. You will be responsible for assessing clients' needs, developing personalized plans, and coordinating resources to help them achieve their goals. This position requires strong interpersonal skills, empathy, and a passion for making a positive impact in the community.
Essential Duties & Responsibilities:
- Conduct initial assessments to understand clients' needs, strengths, and challenges.
- Collect relevant information to determine eligibility for programs and services.
- Develop individualized case plans in collaboration with clients, setting achievable goals and objectives.
- Monitor progress regularly and adjust plans as needed to ensure effectiveness.
- Connect clients with appropriate resources, including but not limited to housing, healthcare, employment, education, and social services.
- Liaise with external organizations and agencies to access additional support for clients.
- Advocate on behalf of clients to ensure they receive necessary services and support.
- Provide emotional support, guidance, and encouragement to clients facing challenges.
- Maintain accurate and up-to-date case records, including assessments, progress notes, and service plans.
- Prepare regular reports on client outcomes, program effectiveness, and other relevant metrics.
- Collaborate with other team members and community partners to enhance service delivery and support systems.
- Participate in case conferences, meetings, and training sessions as required.
Other Duties:
- Adhere to policies related to boundaries with and protection of clients.
- Complete required abuse prevention training (Safe Environment) and others, as required by program.
- Report suspicious and inappropriate behaviors.
- Follow mandated abuse reporting requirements.
- Adhere to job specific abuse risk management responsibilities.
- Adhere to procedures related to managing high risk activities and supervising clients, as applicable.
EDUCATION & TRAINING:
- Bachelor's degree in social work, psychology, counseling, or related field (Master's degree preferred).
- Previous experience in case management, social services, or related field.
KNOWLEDGE, SKILLS & ABILITIES:
- Strong interpersonal skills and the ability to build rapport with diverse individuals.
- Excellent communication skills, both verbal and written.
- Ability to work independently and as part of a team.
- Knowledge of community resources and support services.
- Empathy, patience, and a genuine desire to help others.
- Proficiency in Microsoft Office and case management software (preferred).
Background Check:
This position requires a criminal background check, child abuse/neglect check, and fingerprinting.
Drug Screen:
Pre-employment and random.
Driving Record:
Clear driving record, valid TX driver’s license, required insurance, and reliable transportation.
Health:
Pre-hire TB screen. Other public health screenings or immunizations as indicated by the program.
Language requirements:
Bilingual Spanish/French/Dari/Pashto/Urdu/Farsi/English required.

Pay: $20.00 - $23.00 per hour
