**Overview:** We are seeking a compassionate and skilled Home Health Aide to join our team, dedicated to providing high-quality care and support to patients in their homes. As a Home Health Aide, you will play a vital role in assisting patients with daily living activities, managing their medical conditions, and promoting their overall well-being. **Responsibilities:** - Assist patients with daily living activities, such as bathing, dressing, and grooming, in accordance with their individual needs and preferences. - Administer medications and provide vital signs monitoring. - Prepare meals and snacks as needed, with the understanding that patients may have specific dietary requirements. - Provide assistance with mobility, including walking, transferring, and using walkers or wheelchairs. - Manage patient medical records, including documenting medical information and maintaining confidentiality. - Maintain a clean and organized living environment, adhering to HIPAA regulations. - Develop and implement personalized care plans, in collaboration with other healthcare professionals. - Monitor and report any changes in patient condition, including signs of illness or injury. - Provide emotional support and companionship to patients, helping to alleviate feelings of loneliness and isolation. - Work closely with family members and caregivers to ensure continuity of care. **Experience:** - Previous experience working in a healthcare setting, preferably in home care or assisted living. - Proven ability to provide patient care and support, including experience with senior care and nursing home settings. - Knowledge of medical terminology and the ability to understand medical records. - Familiarity with HIPAA regulations and a commitment to maintaining patient confidentiality. - Experience with patient transportation and mobility assistance. - Strong communication and interpersonal skills, with the ability to work effectively with patients, family members, and healthcare professionals. **Skills:** - Ability to lift and transfer patients safely and comfortably. - Proficiency in medical documentation and the ability to maintain accurate and complete records. - Experience with home community care and a commitment to delivering high-quality care in a patient's home. - Knowledge of HIPAA regulations and a commitment to maintaining patient confidentiality. - Familiarity with assisted living and senior care settings. - Understanding of medical terminology and the ability to interpret medical records. - Experience with patient care and support, including experience with senior care and nursing home settings. - Strong communication and interpersonal skills, with the ability to work effectively with patients, family members, and healthcare professionals.
