STAFF NURSE (LPN/RN)
Function of the Job
Under the direct supervision of the Director of Nursing, as well as, the Administrator, Assistant Director of Nursing and Unit Manager. Responsible to provide direct care to residents in the Center for Health Care (CHC).
Essential Job Responsibilities
1. Follow established performance standards of nursing practices and implement facility policies and procedures.
2. Interpret existing policies and procedures to nursing assistants, residents, families and physicians.
3. Supervise and evaluate all direct resident care on unit assigned and initiate corrective action as necessary prior to notification of Unit Manager.
4. Obtain report from nurse that is being relieved and record sufficient information to implement appropriate follow-up action as necessary.
5. Provide report to nurse coming on duty, including sufficient information for follow-up action if necessary.
6. Conduct resident rounds daily, reporting problems to the Unit Manager or Director of Nursing and initiating corrective action as needed prior to notification of Manager.
7. Identify safety hazards and initiate corrective action.
8. Identify resident problems and emergency situations and initiate immediate "life saving" measures in the absence of a physician, unless otherwise indicated.
9. Follow, supervise and evaluate the implementation of Resident Rights.
10. Participate in facility education programs.
11. Provide clinical supervision of nursing assistants providing direct resident care.
12. Attend staff meetings, in-service classes and committee meeting as assigned.
13. Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident population.
14. Assume responsibility for compliance with federal, state and local regulations on the assigned unit.
15. Participate in the employee evaluation process by providing information on the clinical performance of nursing assistants to the Unit Manager or Director of Nursing.
16. Complete report for facility committees as assigned.
17. Administer and document medications and treatments per the physician’s order and accurately record all care provided.
18. Perform and document comprehensive evaluations of each resident as indicated
19. Develop an accurate Baseline care plan upon admission based on each resident’s comprehensive evaluation.
20. Observe, evaluate and document resident care provided, changes in resident’s conditions and resident’s response or lack of response to care provided.
21. Report changes in resident’s conditions to a Physician, Unit Manager, Director of Nursing, and responsible party and take follow-up action as necessary.
22. Notify physician, when necessary, of changes in residents’ conditions, weight changes, medication errors, medication not available, abnormal lab and x-ray results and any other information as necessary.
23. Follow facility admission procedures and communicate resident admission to other departments, physicians and ancillary service providers.
24. Have knowledge and competency to perform basic nursing skills.
25. Follow facility discharge/transfer procedures, coordinate discharge/transfer with other departments, physicians and ancillary providers and complete documentation.
26. Participate in resident care conferences as necessary.
27. Listen to resident, family or physician concerns and initiate appropriate actions and report concerns to Director of Nursing.
28. Supervise response to resident’s calls for assistance.
29. Count all narcotics, sign for count and exchange keys.
30. Make report to nursing assistants and explain all assignments.
31. Check meal trays to ensure proper diet is served and residents are assisted as necessary.
32. Prepare and administer medications, tube feedings and treatment as ordered.
33. Supervise application and removal of all postural supports and restraints.
34. Order medications as needed, receive medication delivery from pharmacy, check all drugs received, and document proof of medications received, according to facility procedure.
35. Administer PRN medications and treatments as necessary and document effectiveness.
36. Supervise passing of nourishment.
37. Document daily skilled and weekly progress notes as assigned.
38. Consistently work cooperatively with residents, charge nurses, other nursing assistants, staff physicians, families, consultant personnel and ancillary service providers.
39. Personally obtain continuing education relating to the long-term care field of nursing.
40. Observe infection control procedures and adhere to the Antibiotic Stewardship Program guidelines.
41. Responsible for the safety of residents under supervision.
42. Follow Resident’s Rights policies at all times.
43. Observe all facility safety policies and procedures.
44. Come to work in a clean, neat uniform and consistently present a professional appearance.
45. Come to work as scheduled and consistently demonstrate dependability and punctuality.
46. Accept assigned duties in a cooperative manner.
47. Assume accountability of the employees’ handbook and Alliance Code of Conduct.
48. Report to the Supervisor, Director of Compliance or Compliance Hotline, any questionable conduct or suspected violation of regulations or Alliance Community for Retirement Living (ACRL) policy covering the delivery and/or billing of healthcare services..
49. Perform related and special duties as assigned by the Unit Manager or Director of Nursing.
Qualifications
· Current licensure
· Possess organizational, observational, and prioritizing skills.
· Ability to implement interventions for patient care based on standards of nursing practice and principles of nursing care.
· Basic Life Support certification.
