SUMMARY: Care Managers serve as a participant’s safety net. In their role, Care Managers coordinate services and connect participants with resources that can help them with their challenges, whether they be substance use disorders and/or mental health matters along with other personal problems that impact their ability to function normally in everyday life. This position reports to Program Manager.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Core duties and responsibilities include the following. Other duties may be assigned.
· Engages participants in harm reduction and treatment services.
· Provides anti-stigma education.
· Conducts community outreach.
· Initiates referrals and linkages for SUD, MATS, mental and physical health and needed concrete services.
· Facilitates access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational/training opportunities, providers to teach life skills and relevant mental health services.
· Assists in developing natural resources and make contact with social support networks.
· Assists participants in developing goals in areas of need, and assists in developing treatment plans which are assessed regularly; conducts medication and mental status assessments as needed
· Coordinates and monitors services, including tracking of activities in relation to care plan.
· Follows up with referrals for services and regularly makes contact with referral sources for outcome statuses.
· Has regularly scheduled one on one contact with assigned participants.
· Identifies and provides emergency crisis services as necessary; makes immediate assessments and responds according to accepted crisis intervention methods and techniques.
· Documents all participant encounters.
· Maintains comprehensive participant files.
· Provides reports and data to immediate supervisor as scheduled.
· Attends all scheduled case conferences, staff meetings and supervision sessions.

EDUCATION AND/OR EXPERIENCE:
- BS/BA Degree in a human services related discipline and 1-3years’ experience working with applicable
populations.
Experience working with SUD and mental health disorders.
