We have an excellent opportunity for a full-time Service Coordinator in our Children's Long Term Support Waiver Program that we provide on behalf of Milwaukee County. Our Service Coordinators currently work remotely, though home visits and some team meetings are conducted in-person.
Essential Functions
- Establishes positive, helpful relationships with clients and families, service providers and county and state personnel
- Is familiar with and upholds all waiver rules, regulations and deadlines as established by the state and county
- Attends meetings as appropriate, including in-house and community meetings relating to CLTS
- Is familiar with and assists parents in locating community resources
- Facilitates and participates in the development of the Individual Service Plan
- Maintains regular contact with families on the caseload to ensure services are being delivered as described by the ISP
- Maintains all required paperwork as it relates to clients. Keeps documents up-to-date in the proper order in the files
- All other duties as assigned
Required Education/License/Certifications
- Bachelor’s degree in Social Work or related field
- At least one year of professional experience working with children who have special needs
- Must have a valid Wisconsin driver’s license and may not have more than three current minor driving violations and/or one current at-fault accident on record. Auto insurance must include $100,000/$300,000 bodily injury and $100,000 property damage

Job Type: Full-time
Pay: $20.50 - $22.50 per hour
