Lead Case Manager - CalAIM Program
We are seeking an experienced and highly motivated person to join our team as a Lead Case Manager for the California Advancing and Innovating Medi-Cal (CalAIM) program. In this crucial role, you will provide outreach and comprehensive case management services to CalAIM recipients.
Responsibilities:
- Conduct outreach efforts to identify and engage eligible CalAIM recipients in need of case management services
- Develop and implement individualized care plans tailored to each recipient's unique needs and goals
- Coordinate and facilitate access to a wide range of medical, social, and community-based services
- Maintain accurate and up-to-date case records and documentation
- Collaborate with interdisciplinary teams, including healthcare providers, social workers, and community partners
- Advocate for recipients to ensure they receive the necessary support and resources
- Provide education and guidance to recipients and their families on available services and programs
- Participate in case conferences and team meetings to discuss and review case progress
Qualifications:
- Current Licensed Vocational Nurse (LVN) license in good standing HIGHOY PREFERRED
- Experience in case management appreciated, however we are able to train
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations
- Proficiency in computer applications and electronic medical record systems
- Demonstrated ability to work independently and as part of a team
- Organized, detail-oriented, and able to manage multiple tasks and priorities
- Compassionate and patient-centered approach to care
