Job Title: Case Manager

Reports to: Director of Youth and Young Adult Services
The Institute for Health and Recovery (IHR) is a statewide service, research, policy, program development and capacity building agency. IHR designs its services based on an understanding of the impact of trauma. IHR’s CARF accredited treatment programs provide holistic interventions for hard to reach and high-risk individuals and families. IHR’s mission, since our founding in 1989, has been to develop and support a comprehensive continuum of care for families, individuals, youth, and pregnant and parenting people affected by alcohol, tobacco, and other drug use, violence/trauma, mental health challenges and other health issues, while advancing principles of health equity and social justice. Our work is focused on addressing and eradicating inequities in our health care, economic, and justice systems, especially as they disproportionately affect women and people of color. Summary Statement: The Case Manager will support Malden TIP (Teach Inspire Prepare) Program, an innovative middle school-based program (grades 5-8), and IHR’s mission to support high risk youth and their families. The Case Manager will work closely with the Case Management team, including clinical supervisor and school personnel, to deliver targeted prevention and intervention programs to students identified as “high risk” and demonstrating concerning and/or problematic behaviors in school and the community.
Essential Functions:
- Outreach to Salemwood personnel, caregivers, and additional referral sources to provide program information.
- Conduct intakes and needs assessments for program participants.
- Provide case management services to 15-20 students and their families per year.
- Develop, implement, and review Individual Service Plans and review every 6 months.
- Identify community resources to support positive leisure activities including afterschool programming and summer activities.
- Plan afterschool and summer activities for participants.
- Provide individual mentoring to participants and plan and lead developmentally appropriate activities.
- As needed, make referrals to community-based providers.
- Develop, maintain, and update lists of community resources to ensure timely and appropriate referrals.
- Provide case consultation with stakeholders involved in each participant’s case.
- Facilitate psychoeducation groups that promote life skills training and strategic peer involvement.
- Support family service efforts through family education and family nights.
- Actively work with school personnel to provide education advocacy, tutoring and homework support.
- Utilize incentives to engage clients in program activities.
- Work closely with other Malden TIP staff
- Maintain all written reports and records (including electronic health records) accurately and in a timely manner.
- Meet regularly with Supervisor for clinical and administrative supervision.
- Communicate with IHR’s main office in Watertown with cloud-based shared calendars, timesheets and check in with supervisors.

Qualifications/Requirements:.
- Master’s degree in social work, counseling, psychology or related field, required.
- Previous experience working with adolescents/transition-age youth, families, mental health, and substance use issues.
- Previous experience providing treatment at home, school, or in community settings.
- Strong familiarity with assigned catchment area communities and local resources.
- Competency in working with diverse individuals, families, and cultural backgrounds.
- Demonstrated oral and written communication skills.
- Energetic, flexible, and able to take initiative and work independently as well as part of a team.
- Must have regular access to a reliable vehicle and a valid Massachusetts Drivers’ License.
