Program Assistant/Job Coach & Driver

General Responsibilities
- To perform all duties assigned by the Executive Director or Program Director. To follow the policies and procedures of the DAC.
- To become familiar with the individualized program plans for each client served in the Program, particularly each client’s Individual Abuse Prevention Plan and Self-Management Assessment.
- To assist clients in getting to their assigned program area & community sites.
- To see that clients are engaged in scheduled individualized programming that includes meaningful, age appropriate work or activities structured to increase health and skills of daily living.
- To see that clients are safe and part of a nurturing environment.
- To assist clients with tasks related to their personal needs. These include meals, snacks, toileting, adult hygiene pants/pads, and repositioning. Must be able to safely lift at least 50 lbs. Follow the specific guidelines set up by the nurse or program director.
- To perform duties necessary to ensure a clean, safe, and stimulating program area.
- To report situations concerning client need or behaviors to supervisor.
- To report any suspicions of neglect or abuse to administration or the Minnesota Adult Abuse Reporting Center.
- To perform duties assigned by supervisor, treating all clients with respect.
- To request equipment and secure supplies necessary for operation of the assigned program area.
- If assigned: Forklift operation, loading, and unloading of product.
- Driving & Bus Aide as assigned.
- To follow all safety instructions and/or restrictions which may apply to bending, lifting, carrying, and transitioning or transporting clients.
Teaching Responsibilities
- To work directly with clients, teaching skills using modeling, hand-over-hand instruction, verbal commands, verbal prompts, or other means designated by supervisor.
- To work directly with clients teaching appropriate work & social skills.
- To instruct clients in safe practices and safe use of program area equipment.
- To monitor clients as to proper etiquette and appropriate behavior with others.
- To modify the program area (as approved by the supervisor) to allow for better body mechanics and ease of performance.
- To intervene in client conflicts and keep them from becoming crisis situations.
Record Keeping
- To make entries as instructed on appropriate record sheets at the end of each programming session.
- To total figures and submit them to supervisor for inclusion in payroll, billing, and quarterly reports.
