POSITION SUMMARY
The Case Manager provides supportive services for clients in a new residential treatment center for pregnant and parenting women (PPW). This Sober Home, named Bloom House, is a women-only transitional housing / sober living program, and the Case Manager collaborates with the Housing Director and the treatment and program team to meet the needs of mothers and infants in the program. The team sees that all the requirements and services of a CARR-certified organization are fulfilled, and that the residents thrive and graduate to stable, self-sufficient housing. To maintain our Colorado Agency for Recovery Residences (CARR) certification, only women may perform UA testing, therefore only women are allowed to work in this role. Please note that this Sober Home does not allow cigarettes, only vapes.
DUTIES AND RESPONSIBILITIES
Case Management:
· Maintain a case load of up to 9 women; develop individualized service plans for each participant in the program and conduct routine and regular case management as needed per phase and by client need. Case management appointments can decrease over time in the program.
· Work with each client to set goals for healthy individual growth with an emphasis on maintaining recovery that supports the treatment plan; monitor progress in attaining goals.
· Monitor adherence to Homeward Pikes Peak and the Bloom House policies and procedures.
· Assist clients with community engagement and obtaining resources to increase self-sufficiency; assist with enrolling with childcare; coordinate with other community service agencies to access services for clients as needed.
· Transport clients to appointments when necessary.
· Work within a highly collaborative team including the House Manager and relief workers to provide comprehensive transitional housing, in a recovery-focused program.
· Communicate with courts, probation/parole officers and community agencies to obtain prospective clients and complete enrollment of appropriate clients. Receive and work with referrals and applications of new clients.
· Create and maintain written client records and databases, evaluation tools, individual service plans, and adhere to appropriate confidentiality standards relative to data.
· Use the HMIS database to organize client records and data for reporting.
· Interface with agencies, representing Homeward Pikes Peak on behalf of clients, including appear at court and agency hearings and proceedings. Attend client staffing with other agencies and entities with which clients are involved.
· Work with the Homeward Pikes Peak team to review and update Client Handbook, Policies & Procedures, Phase Protocols, and other Bloom House documents.
· Coordinate client services, such as oversee bus pass purchase and distribution, client outings/recreation.
· Ensure clients have access to appropriate professional staffing within various agencies.
· Other duties as assigned.
HOMEWARD PIKES PEAK TEAM RESPONSIBILITIES
· Attend and participate in staff meetings.
· Attend and/or participate or interact with donors, attend community events, or fundraising as requested.
· Integrate with the overall organization and collaborate with the community.
· Produce documentation timely as requested for audits and audit resolutions.
· Maintain office environment to professional standards and display abundant teaming skills.
· Consistently display, adhere to, and promote the company’s Values.
KNOWLEDGE, SKILLS, & ABILITIES
· Associate’s degree in a human services field such as Social Work, Sociology, Human Services, Nursing or a related field.
· Three (3) years of experience in a human services field providing direct client supportive services.
· Education and relevant experience may be interchangeable on a year-for-year basis.
· Lived experience is preferred, with an understanding of challenges related to homelessness, mental illness, and substance abuse.
· Experience working directly with people from diverse socioeconomic and ethnic backgrounds.
· Ability to communicate effectively, perform crisis intervention, and display harm reduction capabilities and trauma informed care.
· CAS (Colorado Addiction Counselor) certification is preferred.
· First Aid / CPR certified, or willing to obtain within 3 months of hire.
· QMAP (Qualified Medication Administration Personnel) certified is preferred.
· Completion of appropriate continuing education requirements every year to maintain licenses and certifications.
· Self-starter and ability to work independently with minimal supervision.
· Strong computer skills required with ability to work with electronic record keeping applications, use of Excel spreadsheets, including PowerPoint and Word.
· Understanding of HIPAA regulations and standards regarding privacy and confidentiality in the provision of therapy services and medical billing.
· Valid Colorado driver’s license, clean driving record, and must have personal vehicle insurance.

PHYSICAL DEMANDS:
· While performing job duties, the employee will be regularly required to sit and stand for the duration of their shift.
· Must be able to move or lift 30 lbs. on a regular basis, and heavier on occasion.
COMPENSATION AND BENEFITS
· The salary for this position is $22.00 - $24.00 per hour, depending on experience.
· This is a full-time non-exempt position.
