Community Mediation Services is looking for a full-time Social Worker for our Mentoring and Advocacy Program (MAAP), who will be responsible for providing counseling services to high-risk youth between the ages of 12 to 21 in the borough of Queens (Jamaica and Far Rockaway). The MAAP Social Worker will manage all incoming program referrals, conduct intake and assessments for all referred youth and match youth to MAAP mentors.
The MAAP Social Worker will provide task supervision for all MAAP Mentors. He/she will oversee the development and execution of all participant individualized goals plans with data entry in ACS and OCFS databases. The MAAP Social Worker reports to the MAAP Supervisor.
Responsibilities:
- Conduct initial intake and youth assessment with youth ages 12-21 and parent/guardian.
- Collaborate with all MAAP Mentors on cases.
- Assist MAAP Mentors in the development of offsite recreation activities, referrals, and advocacy duties.
- Oversee the maintenance of confidential records of each youth.
- Maintaining face-to-face and telephone contact with participants, monitoring re-arrests, and preparing written progress reports for court.
- Make referrals for outside services when needed such as substance abuse, mental health counseling, etc.
- Prepare and submit progress notes, FASPs, enter contacts in PROMIS and other time related material associated with the monitoring of a family.
- Coordinate referrals and linkages within the community for children and families.
- Utilize and manage both the CONNECTIONS and PROMIS applications for caseload accuracy, case assignments and case contacts.
- Log all referrals and intakes.
- Develop and maintain relationships with potential partnering agencies.
- Conduct field visits to provide support to clients in their communities (home, school, medical appointments, etc.).
- Appearing and advocating in court regarding participants’ compliance as necessary.
- Provide coverage for other direct service staff as needed.
Qualifications: A Master’s degree from an accredited college or university. Masters in Social Work, Human Services or Social Sciences (preferred)
- A minimum of two years’ experience in youth work and or youth counseling (preferred).
- Demonstrated ability to engage high risk, inner-city adolescents.
- Good communication skills, both oral and written. Ability to speak professionally to staff, members, parents, groups and community residents.
- Spanish speaking a plus.
- Willing to work in the homes and community.
- Flexible, with availability to work a weeknight and occasional weekends.
- A working knowledge of resources in Queens, especially in the Jamaica and Far Rockaway.
- Computer proficiency in Microsoft Office, Google Docs, Internet Research, and social media
- Highly motivated, with proven capacity to work independently, take initiative, and thrive in high-paced entrepreneurial environment.
