As a Registered Nurse, you will be an essential part of the interdisciplinary team that makes a difference in the lives of our patients every day. You will perform a variety of specialized and highly skilled nursing services, coordinate the treatment plan with the other team members, and communicate with the patient’s physician and family. Care is provided one-on-one with in the patient’s home.

Primary Responsibilities:
· Plan of Care: Perform case assessments and participate in the development of patient care plans. Manage the interdisciplinary care as applicable while promoting continuity of care.
· Supervision: Oversee the care of patients provided by Home Health Aides and Licensed Practical Nurses.
· Education: Provide education to the patient and family on disease management.
· Communication: Effectively communicates with the interdisciplinary team, patient and family regarding all aspects of care.
· Documentation: Complete all clinical documentation accurately and timely to ensure optimal care is provided.
Skills:
· Clinical Expertise: Wound Care Certified and/or Behavioral Healtha Plus
· Excellent Communicator
· Proficient Documenter
· Attention to Detail
Requirements:
· Valid Registered Nurse (RN) Licenses in the State of Virginia
· Minimum one (1) year RN Experience
· Home Care Experience Preferred
· Valid Diver’s License and Auto Insurance
