JOB SUMMARY: Deliver comprehensive nursing care to adults who have a medical or cognitive impairment within the context of a medical day program. Conduct nursing assessment. Monitor client medical conditions, including administration and supervision of prescribed medications. Monitor clinical behavior and nutritional status. Implement infection control procedures. Provide appropriate documentation of these services as required by agency policy. Provide help with activities of daily living.
JOB RESPONSIBILITIES:
1. Demonstrates sound knowledge of medical nursing standards of practice.
2. Performs initial nursing assessment and reassessment of clients according to applicable program
standards and identified tools.
3. Identifies actual and potential health problems, analyzes the information gathered, and formulates nursing diagnosis, making timely and appropriate referrals and follow up for medical needs.
4. Identifies emergent medical problems and intervenes appropriately.
5. Utilizes psychotherapeutic interventions to assist clients in enhancing or improving the ability to manage life stressors.
6. Applies principles of medication administration and storage, demonstrating safe practice and accuracy.
7. Provides ongoing medical education to clients and families especially in relations to; medication management, symptom management, and health education and documents same.
8. Interview the participant and caregivers in order to evaluate the participant's health status and health care need.
9. Maintain the standards of nursing practice including but not limited to: monitoring of identified medical conditions, administration and supervision of prescribed medications and treatments; coordination of rehabilitative services, development of a restorative nursing plan; monitoring of clinical behavior and nutritional status; assisting with the maintenance or redevelopment of the activities of daily living skills; monitoring growth and development; implementing infection control procedures; and communicating findings to the attending physician and family. Provides in-service training to the center staff.
10. Manage medical emergencies.
11. Assist client with Activities of Daily Living, e.g. feeding, ambulating, and toileting.
12. Document the nursing services provided, including the initial assessment and evaluation of the participant's healthcare needs, development of the nursing component of the individualized plan of care, evaluation of the participant's progress in reaching established goals and defining the effectiveness of the nursing component of the individual plan of care.
13. Oversee the development of the initial individualized interdisciplinary plan of care and completes quarterly updates for team.
14. Oversees operation of the program and services in the absence of Director .

15. Alert others involved with the participant's care about changes in status and the need to change the individualized interdisciplinary plan or care.
16. Co-facilitate monthly support group meetings.
17. Participate in quarterly Pharmacy and Therapeutic Committee Meetings.
18. Works in collaboration with dietary consultant to carry out recommendations that meet individual nutritional needs.
19. Adhere to requirements in general Skill Evaluation.
20. Will perform other duties and responsibilities as requested by VHS management without giving up the right to the grievance process.
JOB QUALIFICATIONS: EDUCATION- Graduate of an accredited school of nursing required, BSN preferred.
EXPERIENCE/KNOWLEDGE: One year geriatric nursing.
OTHER:
1. A current New Jersey State Registered Nurse license.
2. Physical: Visual and auditory acuity sufficient to evaluate, treat, and monitor client needs, recognize
clients, and maintain accurate records. Fine motor skills for legible written documentation and use
of keyboard. Ability to work required hours and fulfill job requirements.
3. Maintains CPR and First Aid certifications.
4. Annual Mantoux test.
5. History and Physical every two years.
6. Willingness to learn basic computer and word processing skills.'
