Registered Nurse Care Manager
Position Description
Overview
The Registered Nurse Care Manager reports to the Healthy Starts for Buffalo Project Director with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. They should embody CINQCARE’s core values, including,

Job Responsibilities
The Registered Nurse Care Manager will have the following responsibilities:

Lead the coordination of HS participants’ whole-person care, including medical, behavioral, and health-related social care

Partner with the CHW Supervisor to marshal the project’s Community Health Workers to facilitate non-clinical care interventions, targeted health education, and social support

Conduct clinical assessments using evidence-based tools, under the guidance of the Project Director and Chief Clinical Officer

Prepare participant-centered care plans
- Constituting and coordinating each participant’s interdisciplinary care team

Coordinate and monitor care plan implementation and triage changes in status

Facilitate transitions of care to and from a higher level of care

Lead daily huddles and weekly case conferences with Project Director support

Conduct reassessments and update care plans according to practice standards

Facilitate participant discharges according to HS program guidelines
General Duties
The Registered Nurse Care Manager will have the following duties:

Leadership: The Registered Nurse Care Manager will lead in defining and executing strategies and solutions to create business value, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.
- Strategy: The Registered Nurse Care Manager will establish the business strategy and roadmap: (1) improve outcomes for CINQCARE Family Members; (2) enhance the efficacy of other CINQCARE. business divisions; and (3) develop and deliver external market opportunities for CINQCARE products and services. In establishing the business strategy, the Nurse Care Manager will define and innovate sustainable revenue models to drive profitability of the Company.
- Collaboration: The Registered Nurse Care Manager will ensure that capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.
- Knowledge: The Registered Nurse Care Manager will provide subject matter expertise in the solutions, including determining and recommended approaches, solution deployment and performance evaluation.

Qualifications
The Registered Nurse Care Manager should have the following qualifications:

Education: Registered Nurse degree; NYS RN license required.

Experience: At least 3 years’ patient care management experience, preferably in an obstetric practice setting;

Strong experience with electronic medical records, health information exchanges, coding, and related systems and tools

Excellent written and verbal communication skills and experience communicating and leading interdisciplinary care teams

Comfort working in a diverse, fast-paced environment

Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE’s team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.

Compensation: $77K - $85K
