Job Summary:
Fairview Skilled Nursing Home is seeking a dedicated LPN with specialization in skilled notes to join our skilled rehab team. As an integral member of our healthcare staff, you will play a crucial role in providing exceptional care to our residents, documenting skilled notes accurately, and collaborating with the interdisciplinary team. Your expertise and attention to detail will contribute to the well-being and recovery of our residents in a compassionate and supportive environment.
Responsibilities:
- Conduct comprehensive assessments of residents' physical, psychological, and social needs to develop individualized care plans.
- Implement and evaluate the effectiveness of the care plans, making adjustments as necessary to ensure optimal resident outcomes.
- Monitor residents' progress, documenting and reporting any changes in their condition promptly and accurately.
- Perform skilled nursing procedures, including wound care, medication administration, IV therapy, and other treatments in accordance with established protocols and standards of practice.
- Provide education and support to residents and their families regarding their health conditions, treatment plans, and available resources.
- Collaborate with the interdisciplinary team, including physicians, therapists, social workers, and other healthcare professionals, to ensure coordinated and holistic care.
- Maintain accurate and up-to-date electronic medical records, including skilled notes, progress notes, and other relevant documentation.
- Adhere to infection control protocols and maintain a safe and clean environment for residents and staff.
- Stay updated with current nursing practices, regulations, and industry trends to enhance knowledge and skills in providing quality care.
Qualifications:
- Current LPN license in the state of practice.
- Minimum of two years of nursing experience in a skilled rehab or long-term care setting.
- Specialization in skilled notes/documentation is required.
- Proficiency in conducting comprehensive assessments, developing care plans, and implementing nursing interventions.
- Strong knowledge of wound care, medication administration, and IV therapy.
- Excellent communication and interpersonal skills to collaborate effectively with residents, families, and the interdisciplinary team.
- Detail-oriented with strong organizational and documentation skills.
- Ability to prioritize and manage multiple tasks efficiently in a fast-paced environment.
- Familiarity with electronic medical records (EMR) systems.
- Commitment to providing compassionate and patient-centered care.
