West County Health Centers (WCHC) is a Federally Qualified Health Center (FQHC) and a national leader in transforming community health centers into patient-focused centers of care. WCHC is a physician led and focused organization with a full care team of support for providers. We practice innovative rural medicine in gorgeous West Sonoma County.
For 50 years, WCHC has provided access to comprehensive, quality and accessible healthcare services in the towns and cities of Sebastopol, Occidental, Forestville, and Guerneville, California. We are proud to offer a Wellness Center, Dental Clinics, and a Youth Services Center in addition to our family medicine practice locations. We offer meaningful FQHC work serving diverse populations with the ability to choose your own adventure(s): Latine community focus, reproductive justice, OB, HIV/AIDS, youth health and school clinics, gender affirming care, healthcare for those experiencing homelessness, substance use disorder treatment, wellness services and integrative health.
In the 1980s, during the early years of the HIV/AIDS pandemic as the virus began to impact our communities, WCHC responded by making sure our patients had access to the best possible medical care. As the medical community struggled to address the pandemic, our family physicians became HIV experts, and we developed robust systems with our community partners to care for people living with HIV. WCHC was one of the first community health centers to receive Ryan White Care Act (RWCA) funding through what is now Part C of the RWCA. Our HIV program has become a model and a leader in the integration of HIV care in a primary care setting.
