DEPARTMENT: Mobile Vaccination Clinic
POSITION: Health Coach
REPORTS TO: Program Coordinator
SALARY: $26.00 hourly + benefits
DEFINITION: Under the general supervision of the Program Coordinator, the Mobile Vaccination Program Health Coach is responsible for assisting with the planning, organizing, and implementation of education and training programs to meet the needs of children, adolescents, parents of children, adolescents, and adults encountered during the Mobile Vaccination Clinics.
QUALIFICATIONS:
1. Possession of a degree/certification from an accredited college in education, health education, nursing, or a comparably related field.
2. Preferably one (1) year of recent full-time paid experience working as a Health Educator.
3. Experience working with:
a. Family life education
b. Adults being seen by primary care providers for preventive and chronic health related reasons.
4. Must be well organized with good oral and writing skills and develop written materials for adolescent and adult audiences.
5. Ability to travel to various outside site meetings in a timely fashion.
6. Ability to fluently speak English and Spanish preferred.
7. Effective public speaking skills required.
EXAMPLE OF DUTIES:
1. Assists in the planning, coordination, generation, and distribution of materials for the Mobile Vaccination Clinic
2. Works with community organizations to implement the objectives of the Mobile Vaccination Clinic Health Education.
4. Gathers data required for assessing health-related needs of adolescents and patients.
5. Prepares reports and other written communication as required.
6. Provides health group and individual services to and facilitates information and discussion with children, adolescents, and adults, parents of children and adolescents, patients, community groups, and agency staff.
7. Attends meetings as scheduled.
8. Operates and maintains audio-visual equipment, if needed
9. Ability to establish and maintain effective working relationships with supervisorial personnel, co-workers, subordinates, and the general public.
GENERAL REQUIREMENTS:
1. Attends work regularly and punctually; is readily accessible during work hours to peers; limits absences from own duty stations and limits visits and activities which distract others from their performance of duties.
2. Works cooperatively with immediate supervisor, with department members, and with other Council staff persons, as required
3. Treats all patients / clients / visitors with respect and courtesy.
4. Remains competent in skills required for position.
5. During the initial thirty (30) days of employment, becomes and remains knowledgeable about and in compliance with the Council's and Department policies and procedures in the following areas:
a. Confidentiality of patient / client / fellow employee and supplier records.
b. Patient's rights and responsibilities.
c. Professional and business ethics.
d. Continuous quality and improvement.
e. Safety and security of person and property.
f. Infection control.
g. Hazardous materials and waste.
h. Life safety and emergency preparedness.
6. Maintains a working knowledge about and in continued compliance with all of those areas listed under item (5) throughout your employment with the Council.
7. Provides care appropriate to the age of the patients / clients / visitors served.
8. As a condition of employment, a physical examination and drug-free status is required.
9. Is able to work and/or interact satisfactorily with a workforce and public which is ethnically and culturally diverse.
