Swing Care is a groundbreaking specialty care practice focused on treating fibromyalgia and other medical conditions where central sensitization is a significant factor. We offer telemedicine-based coordinated care, medication management, Stanza, an FDA cleared digital behavioral therapy for treatment of FM symptoms, and individual and group mental health services.
Swing Care Provider Group is an independent professional clinic, administratively managed by Swing Therapeutics, a leading developer of prescription digital therapeutics.
As a Telemedicine Nurse Practitioner, you will be an integral member of the care team who provides comprehensive, virtual medical care to fibromyalgia patients via audio and video calls as well as via written communications. To be successful in this role, you should have a proven ability to provide high-quality, patient-centered care in a remote telemedicine setting. You should understand the value in combining physical, pharmacological, and behavioral-health treatments in helping patients with fibromyalgia better manage their condition. You are a caring, compassionate individual who enjoys helping others and providing a positive care experience on a telemedicine platform. You understand the frustrations of a patient living with a challenging and variable chronic illness, but understand that comprehensive care for this population is best done in an uplifting, motivationally-based, and empowering way.
This is a work-from-home position. You must have a valid Michigan medical license and the ability to work normal Michigan business hours.
Role is for 16-24 hours a week initially as an independent contractor but could expand to full time as patient volume requires.
CONTRACT RATE:
- $50 - $60 per hour, depending on experience and fit for the role
SPECIFIC DUTIES:
- Deliver the human-first, virtual care of Swing Care patients.
- Conduct video, phone, synchronous and asynchronous clinic visits and messaging with patients, including assessment, diagnosis, and formulation of comprehensive (pharmacologic, behavioral, and physical) treatment of fibromyalgia.
- Identify opportunities to collaborate and develop clinical integration opportunities with patients and other health delivery systems to achieve clinical and operational outcomes.
- Provide medical and clinical guidance, support and education to the clinical nurse coordinators and medical assistants and provide ongoing updates and feedback to the Swing Care physicians regarding patient progress and care.
- Ensure compliance with medical and clinical goals through monitoring care management performance.
- Work closely with the Medical Director and other key staff to develop and implement care model enhancements.
- Ensure all work complies with Swing Care’s internal policies and compliance program
QUALIFICATIONS
EXPERIENCE: Five (5) years professional post-NP graduation experience in direct patient care in one of the following areas with experience in treating medically complex patients with multimodal care including pharmacologic and non-pharmacologic therapy:
- Internal Medicine
- Family Medicine
- Pain Medicine
- Physical Medicine and Rehabilitation
- Rheumatology
- Neurology
- Psychiatry
EDUCATION AND LICENSING: Graduation from an accredited nurse practitioner program, current unrestricted license to practice in Ohio, certified by the American Academy of Nurse Practitioners Certification Board, and an active DEA license. Must be willing to expand state licensure to at least 5 additional states. Additional licensure for IL, MO, NV, OH, PA, SC, TN, and TX preferred.
COMMUNICATION SKILLS:
- Excellent written and verbal communication skills with the ability to prepare and present materials if necessary, and to execute and maintain written reports and records.
- Exhibit exceptional interpersonal skills and proper judgment when dealing with key partners, staff and patients.
- Adept at providing medical education to patients regarding pathophysiology of disease and best-practices in multidisciplinary care
- Shows respect and sensitivity to patients living with a frustrating disease.
BACKGROUND: Able to pass detailed background checks according to company policy.
PATIENT PRIVACY: Knowledge of HIPAA compliance and an unwavering commitment to patient privacy.
COMPUTER SKILLS: Comfort with virtual visit technology, electronic medical records and Google Workspace.
TECHNICAL REQUIREMENTS: Broadband internet connection capable of supporting video telemedicine and an appropriate working environment to conduct video-based appointments.
