CSO is looking for a Housing Navigator at Friends of the Homeless (FOH) in Springfield.
This position serves individuals experiencing homelessness that access shelter or other access points in the community, and works to connect them to housing. The role is mobile and able to work on and off-site to meet with homeless individuals in the community.
As a Housing Navigator:
You Will:
- Engage and work with homeless individuals within the coordinated entry system to complete assessments and referrals, obtain needed housing documents, assist with housing applications and connect individuals to supportive services when needed
- Manage caseload, address issues and keep up-to-date client records
- Complete assessments and other paperwork as part of a housing plan, and refer to other supportive services if needed
- Participate in a variety of meetings including staff meetings, Case Conference meetings, and provide regular feedback to supervisor
- Collaborate with a network of community partners including outreach workers, shelter case managers, housing and other service providers, and understand all roles and responsibilities within the coordinated entry system
Your Background:
- Bachelor's or Associate's degree (preferred)
- Both lived experience and relevant direct care experience are valued
- Valid driver's license and reliable transportation
