Community Mental Health Center (Est. 1997) is seeking an Experienced Mental Health Counselor to facilitate psychosocial rehabilitation groups. The Behavioral Health Counselor is responsible for:
- Providing orientation for new admissions
- Designing and facilitating group activities
- Developing individualized treatment plans and treatment plan reviews
- FARS
- Group notes
- Discharge Summaries
- Quality Assurance/Quality Improvement
- Manage persons served records to ensure proper clinical documentation is maintained and meets Florida Medicaid Standards.
- Communication with Caregivers/Loved Ones
- Program Development
The counselor is expected to identify challenging behaviors and to implement behavior interventions through environmental rearrangements, social interactions, and crisis intervention.
The counselor participates in multidisciplinary treatment teams to target individualized treatment plans and assist in other services being provided by paraprofessionals and professionals of our Center.
Educational and Experience Requirements: Must have a Bachelor's Degree in Counseling, Social Work, Psychology, or related Human Services Field. Must have Mental Health counseling experience.
All candidates must hold a valid Florida Driver's License. Must be able to pass a level II background screening.
