POSITION SUMMARY
The Family Life Specialist will be responsible for providing care coordination services for pregnant and interconceptional women, infants, and children to age eighteen months using a social work model of service delivery. Staff will provide culturally aware services to participants to include assessments, referrals, care planning, and discharge with the goal of reducing infant mortality.
POSITION-SPECIFIC DUTIES & RESPONSIBILITIES
- Provide psychosocial case management services for OHS participants in variety of settings to include referrals for health and social service needs
- Screen for eligibility for entitlement programs and facilitate referrals to designated CDHC staff for enrollment
- Work in partnership with CDHC clinical staff and OHS Program Staff to support participant needs/goals
- Utilize the existing case health and social services organizations serving families in North/Northeast Omaha, for the purpose of initiating client referrals
- Develop a plan of service with the participant in partnership with clinical staff, medical provider, health and social service referral source, and/or family as appropriate
- Conduct home visits within the OHS participants home environment in accordance with program requirements
- Provide ongoing assessment of needs and progress toward successful transition, through feedback from the participant, referral sources, and CDHC clinical staff
- Input data and documentation for case files in to administrative dataset, per OHS reporting guidelines and expectations
- Provide assistance as needed in preparing monthly performance indicator reports for OHS
- All other duties as assigned.
POSITION REQUIREMENTS
- Education: Bachelor’s Degree required, Master’s Degree preferred; licensed social worker preferred;
- Experience: Experience in consumer advocacy, case management, working with diverse racial and ethnic groups, data collection, records maintenance and care coordination
- Expertise: Good oral and written communication skills, self-starter, knowledge of project target area; strong organizational skills; multi-tasked and flexible;
- Language: English as a primary language, Karen speaking helpful
- Hours of Work: 8:00 a.m. to 4:30 p.m. some evenings and weekends
- Travel: Must be willing and able to travel; must possess valid State of Nebraska driver’s license and own mode of transportation.
- Exposure: The exposure characteristics described here are representative of those an employee encounters while performing the essential functions of this position. For example, while performing the essential functions of this job, the employee occasionally is exposed to fumes or airborne particles and toxic or caustic chemicals. Additionally, the employee may occasionally be exposed to blood borne and other hazardous chemicals. Finally, the noise level in the work environment can occasionally be quite loud. In all cases personal protective equipment will be provided to the employee in combination with adequate ventilation and other engineering controls to minimize the risk of exposure or other hazardous occurrence.

Pay: $20.36 - $24.43 per hour
