St. Jude Neighborhood Health Centers (SJNHC), a Federally Qualified Health Center serving central and northern Orange County and San Bernardino County, California, is seeking an experienced and talented clinical leader to fill the newly created Advance Practice Supervisor and Provider position. SJNHC is a successful and growing community health center committed to providing the highest-quality primary care to our most vulnerable neighbors. To support our continued success and growth, SJNHC has established this new leadership position. The role will be based in SJNHC's offices in the City of Orange, California, but will require regularly rotating to various sites in Central and Northern Orange County and occasional visit to our San Bernardino County High Desert clinics.
Role and Responsibilities:
The Advance Practice Supervisor and Provider will lead and directly supervise our advance practice providers across multiple sites in Southern California. Responsible for achieving organizational goals and objectives in a manner reflective of our Core Values and Mission. Recruit and retain advance practice providers ensuring adequate coverage across all locations. Train and Mentor advance practice providers in meeting and exceeding quality and productivity goals. Leadership duties account for approximately 40% of time.
General, broad-scope, family practice ambulatory care, with option to provide some pre-natal and pediatrics care. Partner with the CMO and Medical Services Manager to develop and deliver high quality patient care resulting in a high level of quality and patient satisfaction. Be flexible and adaptable, moving practice location as necessary to address patient access challenges. Patient care accounts for approximately 60% of time. Call: Requires occasional after hours phone call coverage.
Essential Job Functions:
- Provide direct preventive, acute, and chronic disease management patient care, all within the approved scope of practice.
- Ensure timely and complete documentation of patient care, review and act upon reports, results, and electronic tasks. Complete and submit correct billing codes for each patient encounter.
- Rotate clinical practice across health center locations to address patient access challenges.
- Work collaboratively with all team members to deliver care of the highest quality to all patients.
- Communicate effectively with specialists and managed care plans to ensure patients receive timely and coordinate health care services.
- Lead the development, maintenance, and implementation of standardized advance practice procedures, scope of care, clinical initiatives and services ensuring a high-quality, safe patient care.
- Mentor, supervise, develop, and hold accountable Advance Practice Providers.
- Recruit and retain Advance Practice Providers.
- Lead Advance Practice Providers in achieving quality and patient satisfaction goals.
- Partner with organizational leaders in the achievement of SJNHC’s strategic goals.
- Complies with regulatory agencies and third-party payor sources (for example: Child Health Disability Program (CHDP), Medi-Cal/Comprehensive Perinatal Services Program (CPSP), NCQA-PCMH, and DHCS).

Qualification Requirements:
Education:MSN-NP/DNP, or MSPA/DPA
License/ Certifications: Licensed NP/PA in the State of California, DEA, Basic Life Support, Valid California Divers License
Experience: 5 or more years of experience providing direct patient care.
- Ability to interface effectively with staff, volunteers, and other physicians/providers.
- Ability to obtain and maintain credentials at St. Jude Neighborhood Health Centers, Medi-Cal, and Medi-Cal MCOs.
- Ability to inspire and lead others in service of the community
Preferred Qualifications:
- Experience providing direct patient care in an outpatient community-based setting.
- Experience leading and supervising others, especially medical providers.
- Demonstrates the ability to read, write and speak English and Spanish without an interpreter.

Pay: $145,000.00 - $175,000.00 per year
