The Case Manager will provide help to pregnant women and children in getting the health care and social services needed. The person occupying this position will teach women how to take care of themselves and their new families. The Case Manager will also provide emotional support when needed and will link women to needed services. The Case Manager will work with the mother during pregnancy and continue to work with the new mother and her family to promote positive parenting skills and well child care. The Case Manager is also responsible for maintaining a caseload of approximately 250 active clients by conducting outreach activities and utilizing the case finding list to obtain names of eligible clients.
