SUMMARY
The Social Worker will complete the psychosocial assessment and meet the patient and family’s needs. The social worker will participate in the coordination of care with the Interdisciplinary Group (IDG) and participate in on-call rotation as needed to fulfill the mission, vision, and heart of Acara Hospice.
QUALIFICATIONS
I. Education: Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; or Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education.
II. Experience: Minimum of one year experience preferred.
III. Skills: The ability to establish and maintain effective working relationships with all team members at Acara Hospice. Working knowledge of community resources. Ability to work with computers.
IV. License: Must possess a valid state driver’s license and automobile insurance and have reliable transportation.
ENVIORNMENTAL WORKING CONDITIONS
I. Works in a routine office environment as well as makes frequent visits to community skilled and assisted level facilities, community events, and patient homes. Noise level may be moderately high, ability to work a flexible schedule and extended hours. Ability to travel occasionally and some exposure to
PHYSICAL AND MENTAL EFFORT
I. Prolonged sitting is required. Requires ability to always handle stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet deadlines and agency needs.
RESPONSIBILITIES OF THE POSITION
I. Provides patients and family members with highly complex education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the Texas State Board of Social Worker Examiners scope of practice.
II. Responsible for developing a written plan of care for each patient, family and caregiver prior to providing care and services. Care coordination among team members is demonstrated through a review of team conference notes, clinical notes, and telephone communication contained in the clinical record.
III. Assists a patient and family with finding alternate means for support for basic life needs.
IV. Conducts patient assessments as required by Medicare Conditions of Participation and documents in the medical record according to the Acara Hospice policy and procedure.
V. Coordinates patient respite stays and general inpatient at local facilities to include contracting, transportation and clinical staff for safe patient transfers to/from facility.
VI. Serves as a liaison with local community resources and their staff to facilitate a smooth transition and placement of the hospice patients.
VII. Assists with admissions, discharges, transferring, arranging alternate levels of care, and attending the deaths of the agency patients as needed.
VIII. Participates in the planning and supports the Hospice Annual Memorial service for families of patients who passed away the past 12 months.
IX. Coordinates with Texas Adult Protective Services when needed to make reports concerning patient safety or to assist in Texas APS investigations regarding patient abuse and safety.
X. Performs other position appropriate duties as required in a competent, professional and courteous manner.
Pay: $28.00 - $33.00 per hour
