SUMMARY: Reports to and works under the supervision of the Director of Nursing. Responsible for ensuring the delivery of quality health care for individuals supported in Agency programs. Provide safe, effective, timely, person centered care to individuals residing at multiple LifeStream residences as assigned. Conduct nursing assessments, coordinate health maintenance and completion of clinical nursing skills per Health Care Providers Orders.

Assures the provision of Nursing and Health Related services for Residential consumers according to all applicable DDS, Mass Health, MRC, DPH and other relevant regulations.
- Responsible for nursing specific medication administration, clinical procedures and documentation for an assigned caseload.
- Review Health Care Provider’s orders for implementation of clinical nursing skills and treatment procedures.
- Conduct full physical nursing assessments and monitor overall well-being of each individual as assigned.
- Assess individual’s medical regimems as assigned to ensure that all individuals are referred for and receive health services, which will maximize health and safety.
- Ensures that individuals are referred for and receive services which assist with achieving set goals and maximize physical and emotional well-being.
- Adheres to DPH regulations for medication storage, administration and destruction.
- Assist in obtaining reports and approval of treatment plans from the individuals’ Health Care Providers.
- Monitor the general health status of all individuals assigned on a day-to-day basis, and attend doctor’s appointments when needed.
- Attend all mandatory meetings and maintain required licensures.
- Adhere to DPH, DMR, MRC commonwealth licensure, LifeStream policies.
- Maintain open and effective communication with DDS/MRC Service Coordinators, peers, supervisors, parents, guardians and community health care providers.
- Implements a health care delivery model, which meets the diverse needs of the individuals, served with an emphasis on consumer empowerment and use of community resources.
- Maintains respectful communication with all individuals served.
- Reports all suspicions of abuse or neglect to DPPC as a mandated reporter.
- Ensure compliance with all dietary orders and restrictions.
- Coordinate hospital admissions and discharges as needed.
- Identify areas of clinical development needed for program staff.
- Conduct LPN required trainings consistent with MAP.
- Act as a nursing resource for staff working in programs and staff education as needed.
- Coordinate health care needs of individuals served and assure that delivery of these services occurs in a timely fashion per Health Care Providers Orders.
- Ensure that all required medical documentation is completed within a timely fashion and according to established guidelines and reflect the progress and interest of each individual served.
- Maintain open and effective communication with DDS/MRC Service Coordinators, parents, guardians, employees, other managers, vendors, and community.
- Effective communication of all programs needs reported to Residential Coordinator and Director of Nursing in a timely manner.
- Report any suspicions of abuse or neglect to DPPC as a mandated reporter.
- Attend all required meetings.
- Perform all other duties as assigned by the Director of Nursing.
QUALIFICATIONS AND REQUIREMENTS:
- Maintenance of an acceptable CORI
- Valid Driver’s license
- Negative drug screen
- Use of personal vehicle for work purposes
- Initial physical exam with mantoux testing. Mantoux screening annually thereafter.
- Commitment to the philosophy of community inclusion
- Maintain flexibility with weekly scheduled hours in order to accommodate program needs.
EDUCATION and/or EXPERIENCE:
- Licensed Practical Nurse with current licensure in Commonwealth of Massachusetts.
- Demonstrated history of quality work performance and teamwork in previous positions
- Proficient in clinical nursing skill performance.
OTHER SKILLS and ABILITIES:
- Excellent, written, verbal, organizational skills; proficient interpersonal relations and communicative skills.
