POSITION DESCRIPTION
Position: Clinical Social Worker
Reports to: General Manager
Position Purpose:
A Clinical Social Worker professionally applies social work methods and theory to the diagnosis, prevention, and treatment of psychosocial dysfunction, impairment, or disability, including mental, emotional, and behavioral disorders for individuals and groups. The main responsibility of a Clinical
Social Worker is to screen medical records, conduct interviews and complete a variety of assessments and treatment plans, set treatment goals based on the consumers individualized needs and conduct group therapy sessions utilizing evidence based models and practices. This position ensures that services are provided using an integrated model of service delivery involving the consumer, family members, the residential service providers and others who might be involved in the consumer’s care.
Principal Responsibilities:
- Assist in the assessment, planning, arranging, and implementing of psychosocial educational workshops/programs.
- Responsible for the content of psycho-educational and counseling programs for both individual consumers and groups of consumers and serves as liaison to community, individuals and groups as appropriate.
- Prepare psychosocial educational materials and create curriculums as needed or required.
- Diagnosing and providing treatment to individuals with mental disorders, as well as various behavioral and emotional disturbances.
- Provides psycho-social assessment and diagnosis of behavioral disabilities with recommendations.
- Recognizing the role of consumer in the treatment planning and facilitating treatment planning meetings.
- Maintains appropriate clinical records and provides written reports and communications.
- Participating in general department meetings, treatment team, and case supervision meetings.
- Coordinate with activity coordinator, activity aides, LPN and other staff members in the provision of services to consumers. Provide referrals for services needed.
- Use tests, measurements, and measurement scales for general and specific assessments as well as objective consumer criteria in the provision of services and evaluation procedures.
- Participation as a resource person in in-service training and planning;
- Provide community service information as an outreach worker as required.
- Ensure the provision of quality services that are responsive to the expressed desires and interests of the consumers served and that support the concept of self-advocacy and protect the rights of consumers.
- Responsible for maintaining a safe workplace by following safety procedures, implementing good housekeeping practices and proper materials handling, enforcing the use of personal protective equipment when necessary and reporting all unsafe acts and conditions.
- Work collaboratively with other agency staff and external customers to enhance communication, maintain positive work relationships, identify the needs of others, provide timely response and promote teamwork for the common purpose of internal and external customer service.
- Ensure the dignity of individuals served by treating them with respect and understanding at all times.
Hiring Specifications
- Master’s degree in social work from an accredited institution. Experience in clinical social work activities required.
- Licensed in the state of Illinois as a Licensed Clinical Social Worker (LCSW).
- Ability to counsel consumers individually and in groups;
- Leadership skills in working with individuals and groups (i.e. initiating individual or group discussion, listening, clarifying and facilitating interactions and sharing of ideas).
- Ability to prepare social and family histories and cooperate on the formulation of reports.
- Skill in communicating concepts and information accurately orally or in writing, including formal statistical reports;
- Self starter, capable of managing staff and give directions to others and work under pressure in certain cases.
- Familiarity with multicultural groups, previous work experience in a multicultural setting and willingness to complete multicultural coursework to supplement training if necessary.
- Knowledge and experience with the documentation and billing process for Medicaid and
Medicare mental health/behavioral health services as outlined in their specific regulatory guidelines.
- Effective communication skills, both written and oral.
- Ability to organize assessment information and inputs from a variety of sources for report writing.
- Proficiency in the use of computer and working knowledge of Microsoft Word and Outlook applications, with good typing skills.
