Job Title: RN Skilled Nursing (Floor Nurse)
Job Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our team as a Skilled Nursing Floor Nurse. In this role, you will be responsible for managing and overseeing the care of patients in a skilled nursing facility. As a key member of our healthcare team, you will provide comprehensive care, support, and treatment to patients with various medical conditions, ensuring their safety, comfort, and well-being throughout their stay.
Responsibilities and Duties:
- Conduct comprehensive assessments of patients upon admission and throughout their stay.
- Develop and implement individualized care plans based on patient assessments, medical history, and physician orders.
- Administer medications and treatments as prescribed by physicians, monitoring patient response and documenting any adverse reactions.
- Monitor and manage patient symptoms, vital signs, and overall health status.
- Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure seamless patient care.
- Provide emotional support and education to patients and their families, addressing their concerns and providing guidance on post-discharge care.
- Perform routine nursing procedures, such as wound care, catheterization, and tracheostomy care, following established protocols.
- Document patient care interventions, observations, and progress, maintaining accurate and up-to-date medical records.
- Respond to emergencies and provide immediate medical interventions as necessary.
- Participate in healthcare team meetings, sharing insights and contributing to the development and improvement of patient care protocols.
Qualifications and Skills:
- Valid RN license in the state of employment.
- A bachelor's or associate's degree in Nursing from an accredited institution.
- At least 2 years of experience in nursing, preferably in a skilled nursing facility or similar clinical setting.
- Strong knowledge of geriatric care, chronic disease management, and wound care.
- Excellent communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare team members.
- Exceptional critical thinking and problem-solving abilities.
- Ability to thrive in a fast-paced and dynamic healthcare environment.
- Current Basic Life Support (BLS) certification.
- Proficient computer skills and experience with electronic medical records.
