PURPOSE OF POSITION:
The Senior Rehabilitation Counselor participates in and/or performs functions related to assessment, counseling, diagnostics, treatment, treatment planning, rehabilitation, intervention and other vocational and supportive services for patients in SUD treatment. Conducts individual and group counseling; assess psychosocial, vocational and social functioning aspects of the individuals 'disability, identify strengths, skills, and abilities and collaborates with the individual on identifying goals and appropriate interventions to help individuals achieve the highest level of vocational and rehabilitation functioning. Supervise subordinate rehabilitation counselors.

SUMMARY OF DUTIES AND RESPONSIBILITIES
- Conducts new admission vocational assessments; provide initial orientation to newly admitted patients, maintain vocational caseload and tracks vocational, rehabilitation, categories for patient availability for employment and other work-related activities.
- Identifies the patient's vocational stage and determines the appropriate level of care necessary for the patient to achieve an optimal level of vocational and rehabilitative functioning.
- Implements and coordinates the treatment and recovery goals and objectives for patients with vocational and rehabilitation needs.
- Monitors and documents the progress of patients' rehabilitation; continually reviews the objectives agreed upon with the patient in regard to vocational and educational goals.
- Develops and manages a program of Vocational/Rehabilitation support systems and activities that promote work readiness, and the development of basic skills necessary to find, get and keep a job.
- Serves as a liaison to community-based vocational and educational resources (i.e. VESID) and functions as a patient advocate to assist with attaining educational and. Employment placement and retention.
- Collects, tracks, monitors and analyzes data; completes and submits OASAS and other regulatory agency reports and surveys.
- Provides coverage for staff in their absence (primary care caseload, assessments, individual and group counseling, family sessions, care coordination, referral and linkage services to individuals and their families
- Shows initiative in developing and participating in community activities, outreach, and educational seminars. Participate in quality and performance improvement activities, including tier reviews, chart reviews, survey readiness activities, case conferences and other patient seminars related to SUD and Vocational/Rehabilitation Services.
