Job Description Summary
The RN Case Manager plans, organizes and directs home health and palliative care, and plans and directs services to meet the needs of patients and their families within their homes and communities.
Essential Job Functions/Responsibilities
- Assesses and evaluates patient’s status.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Responds necessary information to the Director of Nursing
- Monitors the re-certification / re-authorization process
- Provides health care instructions to the patient & family
- Completes, maintains and submits accurate and relevant clinical notes.
- Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
- Maintains patient clinical records, statistics, reports and records for evaluation and reporting of organization activities.
- Attends interdisciplinary group meetings.
Position Qualifications
- CA RN License
- CPR certification
- CA Driver License
- Bilingual Preferred (Vietnamese or Spanish)
- HomeCare HomeBase (HCHB) Experience Preferred
Education/License Requirements:

- CA Driver License (Required)
- CPR Certified (Required)
Job Type: Full-time
Pay: $100,000.00 - $110,000.00 per year
