Position: Case Manager (Fitness in the City)
Department: Family and Community Health Services
Location: l575 Blue Hill Avenue, Mattapan, MA 02126
Reports To: Director, Family and Community Health Services
Summary:
Under the direction of the Director of Family and Community Health Services, the Fitness in the City Case Manager will conduct education, outreach, follow-up, recruitment, evaluations and surveys that will reduce the incidence of overweight and obesity in health center and community pediatric patients.
RESPONSIBILITIES
80% Program Planning, Development and Implementation

Enroll a determined number of overweight or obese patients per year in program as determined by the grant.

Assess readiness for patient to change.

Develop wellness goals with patients and families.

Support patients in meeting their goals through at least 1 visit, phone call or interaction per month.

Refer patients to the nutritionist and track their progress.

Attend quarterly FIC (Fitness In the City) meetings

Develop physical activity opportunities through internal and external referrals to improve the health of participants.

Educate health center staff and providers about available community resources.

Work closely with the assigned pediatric provider who will serve as the liaison for the grant.

Convene quarterly meetings with the providers to discuss case reviews, patient assessments and general awareness of the fitness and nutrition options for obese and overweight patients
20% Program Evaluation and Data Collection

Collaborates with nutritionist to develop, submit, and collect pre and post-test evaluation tools.

Conduct surveys at intake, 12 weeks, 6 months and collect height, weight and BMI data at 1 year and enter all data into REDcap (grantor data base)

Provide a written 6-month summary to Boston Children’s Hospital using the required reporting template.

Work with Boston Children’s Hospital in the qualitative analysis of the program to further evaluate the success and challenges of the Fitness in the City program.

Work with the funder to develop targets and measurable goals to help build the capacity of the health center to address overweight and obesity within the context of the medical home.
Perform other related duties as required or assigned by the Director of Family and Community Health Services.

Requirements:
- High school diploma or GED is required.

At least two years case management, community work or outreach experience is preferred.

Experience working with health care systems, community-based organizations, human services and/or working with families is preferred.

Experience working with diverse cultural and linguistic communities.

Candidate should have experience and/or personal interest in providing support, nutritional guidance and promoting healthy eating.

Excellent written and oral communication and organizational skills are required.

Computer skills required.
