Purpose: Licensed Vocational Nurse/ Registered Nurse functions as the primary clinical support to the physicians and patients who are seen, served, and treated by North Texas Preferred Health Partners (NTPHP). The position reports to the NTPHP location leader. On occasion, Licensed Vocational Nurse/ Registered Nurse may also be asked to perform non-clinical responsibilities to ensure continuity of operations throughout NTPHP.
Duties and Responsibilities:
- Answer patient phone calls and emails, document phone notes and triage patient needs in a timely, professional, and friendly manner
- Confirm scheduled appointments and test reminders for patients
- Room patients and record vital signs at visit, including blood pressure, pulse, weight on every visit; temperatures and oxygen saturation when indicated; and height annually.
- Document patient history, medications and allergy updates in patient chart and maintain medical records. Create new charts in EMR if needed. Enter lab orders for physician
- Scan and import medical records in patient chart. Mail reports to patients, as requested and obtain reports completed in other offices or hospitals. Pull and sort faxes for all physicians. Fill out FMLA, disability, handicap placard, and advance directive forms.
- Carry out verbal orders from physician. Assist physician during physical exams, as needed
- Document charges in electronic medical record for nurse only visits, including EKG
- Administer medications and vaccines plus document in EMR. Pull samples for patients
- Perform in-house procedures and testing, such as EKG, spirometry, ear lavage, urinalysis, flu, strep, mono, A1C, and PT/INR. Coordinate lab draws to outside labs, if required. Perform basic wound care.
- Coordinate care for patients outside NTPHP including prior authorizations, referrals, orders, and appointments for specialists, imaging, physical therapy, home health, hospice, DME. Fax medical records, as needed. Fill out and send preoperative exam forms
- Phone and fax prescriptions and refills in a timely manner
- Address and send birthday cards to patients monthly
- Maintain clean and well-supplied exam rooms and equipment. Empty biohazard containers and report low supplies for ordering. Maintain clean common space areas, including waiting room and workstations.
- Engage with pharmaceutical representatives and familiarize them with sampling process
- Protect patient’s rights by maintaining confidentiality of personal and financial information
- Provide coverage for other clinical team members, as needed, by placing patient’s needs above all others
- Provide backup call on nights and weekends, if required by physician
- Identify need for improvement in office process and recommend solutions
- Other duties as assigned
Experience and Qualifications Required:
- At least two (2) years experience, preferably three plus (3+) years as LVN/RN
- Must maintain a current LVN/RN license
- High school diploma or GED required
- Experience with EMR, eClinical Works a plus
- Computer proficient, typing proficiency, software knowledge
- Able to work with confidential material and follow HIPPA and OSHA guidelines
