Job Title: Healthcare Coordinator – RN

Alternative Supports, Inc (ASI) is seeking a highly organized and proactive individual to join our team as a Healthcare Coordinator. As a Healthcare Coordinator, you will play a crucial role in supervising, training, and evaluating staff members to ensure the medical needs of individuals served are in place.
Position Location: This position will be traveling between our residential programs in Waltham, Belmont, Chelmsford, Tyngsborough, Lowell, and Tewksbury.
Schedule: Monday to Friday 9:00AM to 5:00PM, will be required to flex hours at times to accommodate the needs of the agency
Duties and Responsibilities (These duties are a general summary and not all inclusive)
- Works with the Director of Health Services and other ASI team members to ensure continuity of care.
- Acts as a resource to the team and guides interventions that are health and medically related.
- Monitors on-going health status of program clients.
- Provides oversight and training to the Medication Administration Program.
- Ensures compliance with state and federal regulations.
- Participates in medical aspects of the admissions, discharge, or program transfer process as needed but not limited to health care reviews.
- Under the supervision of the Director of Health Services, assesses, plans, implements, and evaluates healthcare services related to individual needs.
- Assists the Director of Health Services with the development, implementation, and evaluation of health/medical care department goals, operational and medical protocols, and data collection tools.
- Assess and provide individualized education related to clients’ health and disease processes; reinforce wellness and preventative health measures.
- Provides hands-on training and consultation to Support Coordinators and Direct Care Specialists regarding medical issues, procedures and techniques, and health and wellness practices.
- Reviews clinical records on a regular basis to ensure adequate services and supports are provided, including any applicable medical interventions, tests, or preventative care as necessary.
- Attends medical appointments and communicates with providers as needed.
- Communicates healthcare information to all key team members.
- Adheres to the agency’s mission, policies, and procedures.
- Performs other duties as assigned by the Director of Health Services and/or senior management.
Qualifications:
- Must be a currently licensed Registered Nurse (RN) in Massachusetts
- 1-2 years of experience in supervision and training is preferred.
- 1-2 years of nursing experience working with individuals with developmental disabilities is preferred.
- Knowledge of Department of Developmental Services (DDS) regulations is preferred.
- Excellent interpersonal, critical thinking, problem-solving, and communications skills.
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
- Must be able to communicate effectively with others and individuals served.
- Clearance through state mandated Background/Fingerprint Checks
- Possess a valid driver’s license.
- Proficient in Microsoft Office Suite
