Job Overview:
We are currently seeking a compassionate and dedicated Hospice Nurse to join our team. The Hospice Nurse will play a crucial role in providing specialized care to patients in need of end-of-life support. The ideal candidate will have a strong background in hospice palliative medicine and a genuine passion for enhancing the quality of life for patients during their final stages.
Duties:
- Deliver compassionate end-of-life care to hospice patients in various settings, including home care, assisted living facilities, and hospitals.
- Monitor and assess patients' conditions, including vital signs and pain management.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Administer medications, perform venipuncture, and assist with comfort measures as needed.
- Educate patients and their families on hospice care options, disease progression, and symptom management.
- Maintain accurate and detailed patient records using electronic health record systems such as Cerner or Epic.
Qualifications:
- Valid RN license with a background in hospice care or palliative medicine.
- Experience working in a Level I Trauma Center or similar high-acuity healthcare environment is preferred.
- Proficiency in procedures such as venipuncture, spinal taps, and other relevant clinical skills.
- Strong understanding of geriatric care principles and end-of-life support.
- Excellent communication skills with the ability to provide emotional support to patients and families.
- Ability to work collaboratively in a fast-paced environment while maintaining professionalism and empathy.
- Flexibility to work varying shifts as needed to ensure continuity of care for hospice patients.
