Very successful home health agency with offices in Garland and Desoto has immediate opening for LVN home health nurses. The agency had been in home health for more than 25 years in Dallas area with excellent reputation with referral sources including hospital, Rehab facility and physicians.
The agency provide services to acute care clients who needs Nursing, Physical Therapy, Occupational Therapy, Speech Pathology, Home Health Aide, MSW and attendants.
Our Veterans services provide acute care through TRIWEST and Home Maker Services through VA of North Texas.
Our NON MEDICAL HOME HEALTH SERVICES (PHC and Private Pay Attendant services) provide ADL services to client through HHSC (PHC) program.
Our Service Area: Includes Dallas, Tarrant (Fort Worth) county and portions of all nearby counties.
Job responsibilities:
Provide skilled nursing care and teaching the clients per Medicare Home Health standards.
RN provide home health client admissions and re-cert at clients home, case manage LVN and coordinate care with therapist and home health aide.
1. Comply with Medicare PDGM standards and Medicare Advantage client home health requirements.
2. Comply with all HHSC and Medicare employee requirements.
