The Case Manager’s primary purpose is to coordinate care related to clients assigned in their programs which includes, but is not limited to, discharge planning, CFT coordination, communicating with external stakeholders, and managing other aspects of the client’s care. The individual contributor’s performance will be measured by your individual outcomes, achieving your individual targets/goals, your contribution to your overall program team/department and your buy-in to the culture of the organization.

Requirements:
What qualifications do you need?

Must have a valid Driver License and meet any credentialing, licensing, and privileging standards as it pertains to the department you are in.

Education requirement for this individual contributor role is: Bachelor’s Degree in human services or related field

A minimum of 2 years of relevant professional experience
-
Proficient in Microsoft Office Suite application software, excellent written and oral communication skills, ability to work well with different internal and external stakeholders.
