- Tracking preventative care requests. Works with members and CMs to connect members to preventative care providers and schedule appointments
- Registering members for COVID vaccine appointments in community and home
- Troubleshooting issues member experience with medical providers and appointments
- Coordinating care management mailings
- Provides administration and department support for care management team. Assists in completing activities related to different projects/initiatives in support of departmental goals.
- Maintains and supports documentation, logs and data entry to monitor activity and/or outcomes in support of departmental goals as needed.
- Back-up to the Member Service/Transportation department and offer coverage/assistance as needed.
- Performs other duties as assigned.
Specific Knowledge, Skills, and Abilities:
- Proficient in the use of office equipment and the computer; Microsoft Word and Excel.
- Self-directing, good organizational and interpersonal skills.
- Bilingual Mandarin Cantonese required.
Job Type: Full-time
Pay: $21.00 - $24.00 per hour
Expected hours: 40 per week
