Position Overview: As a Field Assessment Case Manager, you will play a pivotal role in evaluating, coordinating, and managing cases in the field. This position requires a dynamic individual with strong interpersonal skills, keen attention to detail, and a passion for helping others. You will work closely with clients, community partners, and internal teams to ensure the effective assessment and management of cases.
Key Responsibilities:
Client Assessment: Conduct comprehensive assessments of clients' needs, including their physical, emotional, and social circumstances, through field visits and interviews.
Case Management: Develop and implement individualized case management plans in collaboration with clients, focusing on goal setting, resource identification, and support services.
Coordination: Coordinate with internal teams, external agencies, and community resources to facilitate access to services such as healthcare, housing, education, employment, and legal assistance.
Qualifications:
- Bachelor's degree in social work, psychology, sociology, or related field (Master's degree preferred).
- Previous experience in case management, social services, or related field.
- Strong communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively in a fast-paced, multidisciplinary environment.
- Knowledge of community resources, social service systems, and relevant laws and regulations.
- Valid driver's license and access to reliable transportation.
