A highly reputable home healthcare provider in Massachusetts is searching for a Clinical Lead Case Manager to oversee its South Shore territories. These territories include Quincy, Braintree, and Weymouth. In this role, you will be responsible for a mixture of management and direct patient care, including assessing patients, educating patients/families, and leading a team of RN Case Managers and Licensed Practical Nurses.
Responsibilities:
- Assist the Director of Clinical Services in overseeing and managing 5 RN Case Managers and 20 LPNs.
- Review nursing notes and clinical records.
- Develop and implement plans of care.
- Perform clinical assessments.
- Educate patients and their families on care plans.
- Monitor the overall experience for patients and providers.
Qualifications:
- Graduate from an accredited nursing program.
- Active RN License
- 3+ years of home health experience
Additional Info:
- Full-time, permanent position
- 40 hours/week, 9a-5p
- Compensation: $130,000-$140,000, depending on experience.
