Responsibilities:
- Supervise the planning, coordination and implementation of a structured activity program that meets the needs of participants and reflects Day Haven’s commitment to excellence, innovation and dementia-specific capability.
- Maintain all required participant records and meet all mandated requirements for essential program elements including assessments, care plans, case notes, authorizations, activity records, attendance records, special services (medications reminders, etc.).
- Document and follow-up on all intake calls and follow-up related to visit days and enrollment to ensure eligibility according to NYS Regulations and the appropriateness of the service in meeting individual needs.
- Maintain communication with all families, managed care organizations (MMLTC), private Long Term Care insurance carriers to meet requirements and to ensure participant needs are being met.
- Complete all required training offered by CPC in order to maintain competency and meet the requirements of various funding sources.
- Supervise and coordinate all activities (including arrival and departure schedules, meals, etc.) so that the needs of participants with dementia are specifically attended to.
- Provide direct supervision to staff to ensure that the goals of maintaining function, overall safety, strength-based individualized programming, behavior management, and social/emotional support are maximized for all participants.
- Coordinate staff scheduling in order to ensure adequate staff.
- Lead or assist with Caregiver Support Group and all special events related to Day Haven.
- Supervise food service and ensure that the program is in compliance with all Senior Meal requirements.
- Ensure that all participant attendance records are accurate, completed and submitted monthly to the billing office; track all authorizations and billing records to ensure timely payment.
- Supervise transportation, ordering/storage of supplies and maintain petty case.
- Other assigned duties as needed to promote the safety of all staff and participants, high quality programming and staff morale.
Qualifications: Bachelor’s degree in Social Work, Gerontology, or Therapeutic Recreation preferred or Associate’s Degree plus relevant work experience required.Must have at least three years supervisory/management experience in senior services or related field.
