Job Title Case Manager - HHS
Reports to Program Manager
Status Full-Time, Non-Exempt
Location Santa Fe Springs
SUMMARY
The Case Manager will travel to multiple locations primarily in the Los Angeles County area to case manage and assist with clients with attainment of housing and recovery support service. The Case Manager’s primary responsibilities are to outreach and engage new clients, coordinate housing, support clients with mental and primary health recovery services, and ensure community integration. The person in this position will use harm reduction model to help maintain the highest quality services and foster collaboration between clients and programs. This person is required work a 40-hour work week and may find it necessary to work some evenings and weekends to meet with clients. This position may be called upon to also support Orange County areas if workflow deems it necessary.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Maintain a caseload of up to 20-25 clients for case management services
- Engage individuals to determine motivation and eligibility for housing
- Complete intake package to submit to Clinical Director for revision prior to enroll the client in HMIS
- Enter notes in HMIS within 24 hours of client contact and document all activities
- Assist clients to find a safe place to live and contact property managers and landlords to set-up appointments to show clients different apartment options
- Develop effective, trusting relationships with clients, with a focus on facilitating independence and maintenance of improve physical and mental health
- Collaborate with clients to develop an individual service plan and savings budget for clients; review and update plan quarterly and upon completion of goals
- Coordinate and facilitate three two-hour sessions per week on job development skills, financial planning skills, and parenting skills
- Coordinate services with partner agencies as needed, and assistance with the completion of forms including Disability and Medi-Cal eligibility when applicable
- Responsible for tracking clients’ progress from enrollment to stabilization
- Assist with tasks pertaining to settling up workshops and programs
- Perform new client orientation, including tour appropriate facilities, review program rules and requirements
- Conduct in-home client visits and model positive attitude
- Maintain client files with vital information (i.e. SSI papers, prove of income, courts papers and etc.). Update client progress on daily basis. Ensure that the needs of participants are being met in a timely manner
- Track program participation, employment and compliance. Notify Program Manager when participants don’t meet the requirements
- Coordinates all services required by clients to complete program.
- Assists clients improving home management skills, family coping parenting skills, self-esteem, anger management, stress management
QUALIFICATIONS
- Current, valid California Driver’s with an acceptable driving record and reliable vehicle
- Able to transport clients in your vehicle
- Bachelor’s degree from an accredited college of university in Psychology, Social Work, Human Services or a related field OR 2 years’ experience providing services to the homeless, highly vulnerable populations.
- Minimum of one-year full-time experience working with the homeless, preferably those living with mental illness and/or substance addictions
- Knowledge and experience in chemical dependency and substance abuse treatment
- Detail oriented with excellent time management, organizational, communication, interpersonal, and computer skills
- Able to work in a high tolerance Harm Reduction model with members who have multiple barriers
- Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision
- Strong interpersonal and team building skills
- Self-starter with the ability to stay ahead of the curve and thrive in fast-paced work environment
- Able to obtain and maintain CPR/1st Aid certification
- Experience using HMIS
WORK ENVIRONMENT
- Field and indoor office environment

- Will be exposed to elements like cold, heat, dust, noise and odor
- May need to bend, stoop, twist, and sit throughout the day
- Regularly required to walk or drive to different local sites throughout the day
- Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds

Job Type: Full-time
Pay: $21.20 - $26.50 per hour
