As an Intake Coordinator Assistant you will play a vital role in providing assistance and support to individuals in need. Duties & Responsibilities:
Managing all aspects of the patient intake process in a timely manner.
Responsible for obtaining accurate referral information.
Establishing and maintaining positive relationships with referral sources, responding to their requests and concerns
Communicate patient data and observations verbally and through recording.
Managing the insurance eligibility verification and authorization processes
Enters and uploads paper & electronic documents into the Healthcare Software
Secures all documentation and forwards to appropriate staff.
Assigns a Clinical Manager and schedules/coordinates all disciplines that were ordered according to the agency process.
Supports and maintains a culture of safety and quality.
Follows up with admitting professional field staff and keeps track of their admissions schedules.
Answering phone calls, handling a variety of administrative tasks within the office environment
Must be detail-oriented, multi-task, and an efficient team player with good computer and typing skills.
Participates in the HHA’s information management and performance improvement programs.
Complies with the HHA’s policies.
Maintains patient confidentiality at all times.
Follow up on missing information, errors.
Work closely with Clinicians (RNs, PTs, OTs and some LVNs) to assess their scheduling availability.
Do your best to accommodate their scheduling needs while also ensuring proper coverage of patient visits.
Requirements:
Ability to work independently and as a team.
Ability to handle stress & work in fast-paced environment with a high level of accuracy.
Strong computer skills, including database management and Microsoft Office with proficiency in Excel
Must pass a background check and drug screening
Language Skills:
Able to communicate effectively in English, and Spanish(optional) verbally and in writing.
Additional languages preferred.
Skills:
Understands regulations governing home healthcare.
